I have just one little question: What about a Hypocrea pulvinata growing on Fomitopsis pinicola? Literature and also ascofrance-data say that the main substrate is Piptoporus (betulinus), only BREITENBACH&KRÄNZLIN (Vol. 1) write "it is possible to find this specie on other Polyporales, [...] especially on Laetiporus sulphureus (WINTER 1887)".

Here are the pics, perhaps I have a mistake in the determination and this is NOT Hypocrea pulvinata:

H. pulvinata is known on Fomitopsis, Thyromyces and Piptoporus according to Rossman (1999).
You should test your specimen in 3% KOH, H. pulvinata becomes red in KOH.
