Diplodia on Vaccinium myrtillus

while examining dead stems of Vaccinium myrtillus for microfungi I have come across what is surely a Diplodia species (i.e. anamorphic Botryosphaeriaceae) . . . as workers in the 'coelomycetes' know well, with an unfamiliar fungus which is not in any of the usual books, ones first move is to search IndexFungorum with the genitive of the host genus, i.e. "Diplodia vaccinii"
sadly the link given for Diplodia vaccinii on IF draws a blank - or rather an incorrectly repeated page instead of the one intended - http://www.librifungorum.org/Image.asp?ItemID=71&ImageFileName=SyllogeFungorum11-520.jpg - you will see that it takes you to page 517 - not 520, which is missing!
the original description of Diplodia vaccinii was published in Revue mycol., Toulouse 9: 163 (1887) . . . if anyone can let me have that reference (or point me to where it can be found online) I would be extremely grateful - the conidia are (12)14-22µm long
cordialement
